It had been proven very useful in assisting organization to achieve its target. It is also well-known for its simple features and low cost of implementation. Many had labelled that checklist is a great tool that helped them to achieve their target. It is often heard that in some occasion, organization did produce defects. Some of this defect had prevention steps that already mentioned in the checklist. This raised a major question among its practitioner. They questioned if checklist is already efficient. Some of them also question if it is fool prove. Therefore, there are immediate needs to find the solution and source of these defects.
